Step 1. Secure a copy of the Affidavit of Warranty form either from the LTO website or from any of the LTO offices in the Philippines. You may also request a copy from the LTO officer. 

Step 2. At the top of the form, fill in the following information: 

  • Date: You need to write the current date of filling out the form. 
  • Republic of the Philippines: You need to write the name of the country. 
  • Regional Trial Court: You need to write the type of court applicable in your area. 

Step 3. Fill in the details of the Affiant or the person giving the warranty. This information includes the following:

  • Full Name: You need to write the full name of the person or individual giving the warranty. 
  • Age: You need to write the age of the person. 
  • Citizenship: Write the nationality of the person (Filipino or others). 
  • Civil Status: Write the marital status of the person (single, married, widowed, etc.). 
  • Address: Write the complete address of the person. 
  • Driver’s License No.: Write the driver’s license number of the person.

  • LTO Exam No.: If you have undergone an LTO examination, write the examination number provided to you. 
  • Date and Place: Write the date and location where the examination was conducted.

What is LTO Quezon City? 

When talking about LTO Quezon City, you can refer to either one or all of the fifteen (15) LTO branches located in the city. These branches all serve the same purpose: to offer faster and more accessible LTO services to the residents of Quezon City and its neighbors. 

The LTO Quezon City branches are as follows:

LTO E-Patrol Unit

The LTO E-Patrol Unit stationed at the LTO Compound in Quezon City was launched by the agency to offer off-site operations, mostly in key public and private offices in the city. This initiative was intended to help drivers avoid paying penalties or facing legal repercussions when they fail to adhere to the rules and regulations implemented by the agency. 

Diliman District Office

Just like the other LTO district offices, the LTO Diliman District Office is an office in charge of enforcing land transportation rules and regulations, issuance of permits and licenses, and registration and inspection of motor vehicles within the Diliman area in Quezon City.

La Loma District Office

The LTO La Loma District Office is yet another extension of the LTO and its services, located in Biak-na-Bato, Sta. Mesa Heights in Quezon City 

Quezon City District Office

The LTO Quezon City District Office is also another branch of LTO, located in P. Tuazon Ave. in Cubao. This district office is just one of the many LTO offices in the city meant to bring the LTO services closer to the transacting public.

Quezon City Extension Office

The LTO Quezon City Extension office was designed to be an extension of either one of the LTO offices in Quezon City. Located on 20th Ave. in Cubao, Quezon City, this extension office was created to eliminate the long queues and the overcrowding of license applicants in the district offices in Quezon City. 

Quezon City Licensing Center

The LTO Quezon City Licensing Center is an office especially created to offer fast and efficient LTO services for a more convenient new driver’s/conductor’s license applications or any license transactions with examination, especially for the senior citizens, persons with disabilities (PWDs) pregnant, lactating women, and diplomatic officials.

QCLC-Renewal Section (QCLCRS)

Just like QCLC, the QCLCRS was created as an extension of the district offices and to offer fast and convenient services to the public. It specifically caters to transactions like all student permits, license renewal and/or duplicate transactions.

LTO DLRO Ali Mall 

The LTO also offers a one-stop shop, in the form of the LTO Driver’s License Renewal Office (DLRO) in Ali Mall. It is meant to offer the same LTO services and all forms of assistance that the public needs related to LTO services and concerns.

LTO DLRO Eastwood

Like any regular DLRO, LTO Eastwood City is yet another one-stop satellite office opened by the agency to cater to applications for new student license permits and driver’s license renewals.

LTO DLRC Ever Gotesco Commonwealth

The LTO Driver’s License Renewal Center in Ever Gotesco Commonwealth is another satellite office set up by the agency. It houses three (3) working stations: for Evaluator, Photo/Sig, Cashier and Two (2) Dermalog Station with 1 Laser Engraver all meant to extend the LTO services to the transacting mall-goers and the people living in the surrounding areas. 

LTO DLRO Fisher Mall

As with all the other (or most) government offices, LTO DLRO Fisher Mall officially opened in 2019 with the convenience of the public in mind. It was meant for people who wish to enjoy the convenience of renewing their driver’s licenses and vehicle registrations and is located on the upper basement of Fisher Mall in Quezon Ave.

LTO DLRO Robinsons Novaliches

With the purpose of making driver’s license renewals faster and more convenient, the agency also opened a DLRO on the 3rd level of the main mall at Robinsons Place in Novaliches. 

LTO DLRO SM North Edsa

Though they offer limited service types, the DLRO in SM North EDSA is well-known for fast, convenient, and accessible license renewal services to people who work behind the wheels. This DLRO was opened on the lower ground floor at the Government Service Express Center in SM North EDSA Annex in Quezon City in 2019.

LTO DLRO SM Novaliches

An LTO DLRO was also opened in 2020 on the 2nd floor, at the Wellness Zone of SM Novaliches located at Quirino Hi-way in Novaliches, Quezon City for the convenience of the transacting public in the northern part of Metro Manila.

LTO DLRO UP Town Center

To decongest and shorten the queues in other LTO branches in Quezon City, the agency also opened a DLRO in UP Town Center, a new mall opened along Katipunan Ave., right along the same street as some of the most expensive colleges in the Metro.

Where is Quezon City

Quezon City, also known as the City of Quezon and Q.C., is the most populous city in the Philippines. In 2020, it was already housing a population of 2,960,048 people. 

Originally intended to be the national capital of the Philippines and a replacement for the overcrowded Manila City, several barrios were carved out from the neighboring cities of Caloocan, Marikina, San Juan, and Pasig and attached to eight (8) vast estates to form the city. It was officially proclaimed as the national capital on October 12, 1949, prompting several government departments and institutions to move out of Manila and settle into the new capital city. If in case you fail, then please take note of the following guidelines that applies to both the conductor’s and the driver’s license applications:

  • If a driver’s or a conductor’s license applicant is unable to meet the passing rate, retaking the test is allowed one month after the first attempt.
  • If unable to meet the passing rate on the second attempt, a driver’s or a conductor’s license applicant can retake the exam and apply for a driver’s or a conductor’s license within a year.
  • If unable to meet the passing rate on the third attempt, a driver’s or a conductor’s license applicant has to wait for two years before they can be allowed to retake the test and re-apply for a license.
